Location: St Albans, Vermont

Contract Type: Travel Contract

Shift Type: Tuesday - Saturday, 8:00 AM - 4:00 PM with on-call rotation required

Hours per Week: 37

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Perform comprehensive patient assessments and develop individualized care plans
Complete OASIS documentation accurately and in a timely manner
Manage a caseload as case manager, coordinating with the interdisciplinary team
Deliver skilled nursing interventions including IV therapy, wound VAC, and complex wound care
Educate patients and caregivers on medications, treatment options, and disease management
Coordinate safe discharges and transfers, ensuring all paperwork is in order
Participate in monthly peer review audits and case conferences to ensure quality care

Qualifications

Active Registered Nurse (RN) license in Vermont or compact state
Minimum 2 years of recent home health nursing experience
Strong knowledge of OASIS documentation and home care regulations
Proficiency in IV therapy, wound care, and wound VAC management
Excellent communication and case management skills

Knowledge & Skills

Patient assessment and care planning

Case management and patient education
Use of McKesson or similar EMR systems
