Roles & Responsibilities
- Provide compassionate, skilled nursing care to patients in their homes, helping them remain safe and independent
- Administer medications and treatments per the care plan and document all care accurately
- Perform focused assessments and communicate changes in patient condition to the care team
- Complete Trinity-specific competency testing and skills checklists to maintain high standards
- Educate patients and caregivers on disease management, medication administration and safety at home
- Maintain strict infection control and safety practices, including respirator use and fit testing
- Coordinate with interdisciplinary team members to ensure seamless, patient-centered care
Qualifications
- Current, unencumbered LPN license (Connecticut required or eligible for verification)
- BLS (Basic Life Support) certification required
- Home health experience preferred and valued
- Ability to complete Trinity specialty competency testing and Trinity skills checklist
- Willingness to complete required health screenings: 2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19 and influenza vaccination as applicable
- Successful completion of Trinity 10-panel drug screen, respirator evaluation and fit test
- Reliable transportation and ability to travel within assigned service area
Education
- Completion of an accredited Practical Nursing program and active LPN licensure
Knowledge & Skills
- Solid clinical assessment and wound care skills appropriate for LPN scope
- Strong documentation and charting skills with attention to detail
- Comfortable working independently in patient homes and managing priorities
- Excellent communication and patient/caregiver education abilities
- Time management and route planning to deliver timely, efficient care
- Familiarity with infection control, medication administration and safety protocols
