Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents
• Assess and monitor resident conditions and report changes promptly
• Administer medications and treatments safely and accurately
• Document care thoroughly and maintain clear clinical records
• Collaborate with an interdisciplinary team to create person-centered plans
• Educate residents and families about care, medications, and safety
• Respond to urgent needs and escalate concerns to ensure patient safety

Qualifications

• Active LPN or RN license in New York required
• Current BLS (CPR) certification required
• Prior experience in long-term care, skilled nursing, or similar settings preferred
• Strong communication skills and a team-oriented attitude
• Ability to pass background check and drug screening as required

Education

• Completion of an accredited LPN or RN nursing program
• State licensure (LPN or RN) as required by New York

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Comfortable with bedside nursing tasks, assessments, and care planning
• Familiarity with infection control and safety procedures
• Proficiency with electronic health records is a plus
• Excellent time management and prioritization in a busy care setting
