Roles & Responsibilities

• Provide high-quality physical therapy services to patients in their homes
• Implement and adjust individualized treatment plans to help patients regain mobility and independence
• Document patient progress accurately in OASIS and Cerner EMR
• Coordinate care with a multidisciplinary team including OT and Speech therapists
• Educate patients and families on safe exercises and fall prevention
• Travel to your assigned geographical region to deliver compassionate care

Qualifications

• Active Physical Therapy Assistant license in California
• BLS certification required
• Minimum 2 years of experience as a PTA
• Experience with OASIS documentation is required
• Home health experience preferred but not required
• Ability to work with a variety of diagnoses including ortho, strokes, and spinal injuries

Knowledge & Skills

• Proficiency with Cerner EMR
• Understanding of OASIS federal documentation requirements
• Knowledge of home health care safety and infection control
• Strong communication and patient education skills
• Ability to work independently and manage a geographic assignment
