Roles & Responsibilities

• Evaluate patients' physical therapy needs and develop personalized treatment plans
• Implement therapeutic exercises and modalities to improve mobility and function
• Monitor patient progress and adjust treatments to achieve optimal outcomes
• Educate patients and families on home exercise programs and injury prevention
• Collaborate with the interdisciplinary team to ensure comprehensive, patient-centered care
• Maintain accurate and timely documentation of patient care

Qualifications

• Active Physical Therapist license in Pennsylvania (or willingness to obtain)
• Current BLS/CPR certification
• Graduate from an accredited Physical Therapy program
• Ability to provide state or federal issued identification for facility badging
• Adaptability to work in a skilled nursing/rehab environment
• New graduates welcome to apply

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Proficiency with therapeutic modalities and rehabilitation techniques
• Excellent communication and interpersonal skills for patient interaction
• Comfort with electronic medical records (EMR) documentation
• Ability to work independently and as part of a team
• Flexibility to transition to various patient care settings
