Roles & Responsibilities

• Assess patient functional status and design individualized treatment plans to restore mobility and independence
• Deliver evidence-based therapeutic interventions and progress patients toward measurable goals
• Collaborate with nursing and interdisciplinary teams to coordinate care and ensure continuity
• Document patient evaluations, treatments and progress clearly and timely in the medical record
• Educate patients and families on home programs, safety and strategies to maintain gains
• Participate in team meetings and quality initiatives to improve patient outcomes and clinic processes

Qualifications

• State Physical Therapist license (ability to obtain New Hampshire license required)
• Doctorate or degree in Physical Therapy (DPT preferred; MPT or related degrees considered)
• Current BLS/CPR certification or willingness to obtain
• Clinical experience in adult rehabilitation or related settings preferred
• Strong communication skills and a patient-centered approach
• Ability to perform physical demands of the role safely and effectively

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in Physical Therapy considered

Knowledge & Skills

• Proficient in musculoskeletal and functional assessment techniques
• Skilled in therapeutic exercise, manual therapy and modality use
• Knowledge of documentation standards and outcome measurement tools
• Excellent interpersonal skills for patient education and team collaboration
• Time management and caseload prioritization to optimize patient flow
