Roles & Responsibilities

• Conduct comprehensive patient evaluations to develop individualized treatment plans
• Implement daily therapeutic treatments to improve mobility and function
• Collaborate with the rehab team to ensure seamless patient care
• Complete timely and accurate documentation for discharges and progress notes
• Maintain a high standard of patient satisfaction and clinical excellence

Qualifications

• Active Physical Therapist (PT) license in New Mexico
• Doctorate in Physical Therapy (DPT) required
• Prior skilled nursing facility (SNF) experience strongly preferred
• Minimum 1-3 years of professional experience as a PT
• Ability to meet productivity standards (90%) with a positive attitude

Education

• Doctor of Physical Therapy (DPT) degree from an accredited program

Knowledge & Skills

• Expertise in evaluations, treatments, and discharge planning
• Strong communication and teamwork skills
• Proficiency in documentation and electronic medical records
• Ability to adapt to a fast-paced SNF environment
