Roles & Responsibilities
- Conduct comprehensive patient evaluations to develop individualized treatment plans
- Implement daily therapeutic treatments to improve mobility and function
- Collaborate with the rehab team to ensure seamless patient care
- Complete timely and accurate documentation for discharges and progress notes
- Maintain a high standard of patient satisfaction and clinical excellence
Qualifications
- Active Physical Therapist (PT) license in New Mexico
- Doctorate in Physical Therapy (DPT) required
- Prior skilled nursing facility (SNF) experience strongly preferred
- Minimum 1-3 years of professional experience as a PT
- Ability to meet productivity standards (90%) with a positive attitude
Education
- Doctor of Physical Therapy (DPT) degree from an accredited program
Knowledge & Skills
- Expertise in evaluations, treatments, and discharge planning
- Strong communication and teamwork skills
- Proficiency in documentation and electronic medical records
- Ability to adapt to a fast-paced SNF environment
