Roles & Responsibilities
- Evaluate residents' physical therapy needs and develop personalized treatment plans
- Implement therapeutic exercises and interventions to improve mobility, strength, and balance
- Collaborate with physicians, nurses, and rehab team members to deliver coordinated care
- Document patient progress and modify plans to achieve optimal outcomes
- Educate patients and families on safe movement, fall prevention, and home exercise programs
- Provide compassionate, patient-centered care that supports residents' quality of life
Qualifications
- Active Physical Therapist (PT) license in Virginia or eligibility to obtain one
- Doctor of Physical Therapy (DPT) degree
- New graduates are welcome - any experience level accepted
- Previous SNF or geriatric rehab experience is a plus but not required
- Strong communication skills and a collaborative spirit
- Reliable, compassionate, and committed to patient care
Education
- Doctor of Physical Therapy (DPT)
Knowledge & Skills
- Knowledge of physical therapy assessment and treatment techniques
- Understanding of skilled nursing facility regulations and documentation practices
- Skill in developing and modifying therapeutic exercise programs
- Ability to work effectively with older adults and interdisciplinary teams
- Strong clinical reasoning, problem-solving, and organizational skills
- Familiarity with electronic health records and therapy documentation
