Roles & Responsibilities

• Evaluate patients to identify functional limitations and design individualized treatment plans
• Deliver skilled therapeutic interventions to improve mobility, strength, and independence
• Document patient progress clearly and timely in the medical record
• Collaborate with nurses, therapists, and care teams to coordinate comprehensive care
• Educate patients and families on home programs and safety strategies to support long-term outcomes
• Monitor and maintain therapy equipment and ensure a safe treatment environment
• Contribute to quality initiatives and help improve patient experience and clinical outcomes

Qualifications

• Current Physical Therapist license in Wisconsin or eligibility to obtain licensure
• Graduation from an accredited Physical Therapy program (DPT preferred)
• Basic Life Support (BLS) or CPR certification
• Comfort working in a skilled nursing/long-term care environment preferred
• Strong communication and teamwork skills with a patient-centered approach
• Reliable transportation and willingness to travel between assigned sites as required

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor or Master in Physical Therapy considered

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and mobility training
• Knowledge of documentation best practices and electronic medical records
• Experience with geriatric rehabilitation and fall prevention strategies
• Ability to communicate clearly with patients, families, and interdisciplinary teams
• Good clinical reasoning, time management, and organizational skills
