Candidate Pay Rate: $56 per hour

Roles & Responsibilities

• Provide skilled physical therapy services to patients in their homes, helping them regain independence
• Develop and implement individualized treatment plans to improve mobility and function
• Educate patients and families on exercises and techniques for ongoing recovery
• Collaborate with healthcare team to ensure comprehensive care
• Document patient progress and adjust treatments as needed

Qualifications

• Current Physical Therapist license in Iowa or compact eligibility
• BLS certification required
• Experience in home health or outpatient setting preferred
• Epic experience preferred but not required

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Excellent communication and patient education abilities
• Ability to work independently and manage a flexible schedule
• Familiarity with electronic health records, especially Epic
