Roles & Responsibilities

• Evaluate patients' functional abilities and create individualized treatment plans
• Deliver hands-on therapy and progressive exercise programs to maximize recovery
• Monitor progress and adjust interventions to meet measurable goals
• Document skilled therapy thoroughly and timely in the electronic medical record
• Collaborate with nursing, OT, SLP, and case management to coordinate care
• Educate patients and families on home programs, safety, and fall prevention
• Support transitions of care and provide clear discharge planning

Qualifications

• Current PA Physical Therapist license or eligibility to obtain state licensure quickly
• Doctor of Physical Therapy (DPT) or equivalent PT degree (credentials must meet facility requirements)
• BLS (Basic Life Support) certification required or ability to obtain upon hire
• Experience in skilled nursing, inpatient rehab, or post-acute settings is preferred
• Strong clinical reasoning, excellent communication, and a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred
• Bachelor of Science in Physical Therapy acceptable where applicable

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, and mobility training
• Experience with gait training, transfers, and assistive device progression
• Proficient with documentation in electronic medical records and outcomes tracking
• Knowledge of fall prevention, dysphagia precautions (when relevant), and safe patient handling
• Ability to work effectively within an interdisciplinary team and mentor support staff
