Location: Ada, Oklahoma

Start Date: 06-15-2026

Contract Type: Travel Contract

Shift Type: 8 hr days, 35 GWW, No weekends or on-call required

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Evaluate and rehabilitate patients with physical functioning disorders and/or wound care
Develop and implement plans of care following regulatory and clinical practice standards
Administer various procedures as part of the rehabilitation plan
Assess patient progress and adjust treatments accordingly
Provide consultation and counseling to patients and families

Qualifications

1 year of PT experience
OK State License
LTC or Rehab experience
Level II experience (not just Level I observation experience)
