Candidate Pay Rate: $86 - $60.2 per hour

Roles & Responsibilities

Evaluate residents and develop individualized occupational therapy treatment plans that promote independence
Deliver therapy focused on activities of daily living (ADLs), mobility, and functional skills
Implement therapeutic exercises and activities to improve coordination and upper extremity function
Monitor patient progress, adjust treatment approaches, and document measurable outcomes
Collaborate with physicians, therapists, nursing staff, and interdisciplinary teams to coordinate care
Maintain accurate and timely clinical documentation and communicate care plans clearly
Educate residents and caregivers on adaptive techniques, safety strategies, and meaningful home programs

Qualifications

Active California Occupational Therapist license (required)
Degree in Occupational Therapy from an accredited program (required)
Previous SNF, rehabilitation, or geriatric therapy experience preferred
Strong clinical assessment and patient care skills
Ability to work effectively within a multidisciplinary healthcare team
Excellent communication and documentation abilities

Education

Degree in Occupational Therapy from an accredited program

Knowledge & Skills

Expertise in ADL training and functional rehabilitation
Skilled in patient evaluation and treatment planning
Strong clinical assessment and patient care techniques
Effective communication with residents, families, and clinical teams
Accurate and timely clinical documentation practices
Familiarity with common SNF/rehab workflows and interdisciplinary coordination
