Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents and patients
• Administer medications and treatments as prescribed, and monitor patient responses
• Assess, observe, and document patient conditions, reporting changes to the care team
• Collaborate with registered nurses, physicians, and other healthcare professionals to deliver individualized care
• Assist with daily living activities and support patients' physical and emotional well-being
• Maintain accurate and timely patient records and ensure compliance with facility policies and regulations
• Educate patients and families on care plans, medications, and health management

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
• Current BLS/CPR certification from the American Heart Association or comparable organization
• Strong clinical assessment and critical-thinking skills
• Ability to work a flexible schedule, including nights when needed
• Excellent communication and interpersonal skills
• Prior experience in long-term care, skilled nursing, or geriatrics is a plus

Knowledge & Skills

• Knowledge of nursing principles, medication administration, and patient care standards
• Skill in using electronic health records (EHR) and documentation systems
• Ability to remain calm and effective in fast-paced or high-pressure situations
• Strong attention to detail and organizational skills
• Compassionate, patient-centered approach to care
• Ability to work collaboratively within a multidisciplinary team
