Roles & Responsibilities

• Provide compassionate, patient-centered care to residents and patients
• Administer medications and treatments as ordered by physicians
• Monitor and document patient vital signs and report changes to the healthcare team
• Collaborate with nurses, physicians, and other care team members to ensure quality outcomes
• Educate patients and families on care plans, medications, and health management
• Maintain accurate patient records and follow all facility policies and regulations

Qualifications

• Active and unencumbered LPN or RN license in Pennsylvania
• Current BLS/CPR certification required
• Ability to work flexible per diem shifts, including nights
• Strong clinical assessment and critical thinking skills
• Previous experience in a healthcare or long-term care setting is preferred

Knowledge & Skills

• Medication administration and dosage calculation
• Patient assessment, vital sign monitoring, and documentation
• Electronic health records (EHR) and charting systems
• Infection control and safety protocols
• Effective communication and teamwork
• Compassionate and empathetic patient advocacy
