Roles & Responsibilities

• Provide compassionate direct patient care under the supervision of an RN or physician
• Administer medications and treatments with precision and care
• Monitor patient vital signs and report changes to the care team
• Assist residents with daily living activities to promote independence
• Document patient care accurately and timely in electronic health records
• Collaborate with the healthcare team to ensure high-quality outcomes

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania
• BLS certification is preferred
• Experience in long-term care or skilled nursing is a plus
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Knowledge & Skills

• Excellent clinical assessment and critical thinking skills
• Proficient in medication administration and documentation
• Familiarity with electronic health records (EHR) systems
• Patient-centered approach with a focus on comfort and safety
• Adaptability in a fast-paced healthcare environment
