Roles & Responsibilities

• Deliver compassionate, patient-centered nursing care and build trusting relationships with residents and their families
• Administer medications, treatments, and IV therapies accurately while monitoring for side effects
• Perform comprehensive patient assessments and promptly communicate changes to the care team
• Collaborate with interdisciplinary teams to create and update individualized care plans
• Maintain meticulous and timely documentation to support quality care and compliance
• Educate patients and families on health management, medication, and follow-up care

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Hampshire
• Current BLS/CPR certification
• Must be local to the Milford, NH area (this is a local contract)
• COVID-19 vaccination or approved exemption required
• At least one year of experience in a skilled nursing, long-term care, or similar setting (preferred)
• Strong clinical assessment and medication administration skills

Knowledge & Skills

• Proficient in medication administration and dosage calculations
• Skilled in electronic health records (EHR) and documentation
• Knowledge of geriatric care and common age-related conditions
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a nursing team
• Compassionate, adaptable, and detail-oriented
