Roles & Responsibilities

• Provide compassionate direct patient care including medication administration, wound care, and assistance with daily living activities
• Monitor patient vital signs and condition, reporting changes to the healthcare team promptly
• Document nursing assessments, interventions, and patient outcomes accurately
• Collaborate with registered nurses, physicians, and other team members to create and implement effective care plans
• Educate patients and families about care routines, medications, and health maintenance
• Maintain a safe, clean, and supportive environment that promotes patient dignity and well-being

Qualifications

• Current and valid Licensed Practical Nurse (LPN) license in New Mexico
• Basic Life Support (BLS) certification from AHA or equivalent
• COVID-19 vaccination or approved medical/religious exemption per facility policy
• Previous experience in long-term care, skilled nursing, or geriatrics is a plus
• Must be local to Belen, NM, or able to work as a local contract professional
• Ability to pass a background check and drug screening
