Roles & Responsibilities

• Provide compassionate personal care and assist patients with activities of daily living (bathing, dressing, toileting)
• Monitor and record vital signs and report changes in patient condition to clinical staff
• Assist with safe patient transfers and mobility to promote independence
• Maintain accurate documentation and communicate effectively with the care team
• Follow infection control and safety protocols to protect patients and staff
• Support a warm, respectful environment that preserves patient dignity

Qualifications

• Active LNA / Licensed Nursing Assistant certification (VT or eligible to practice in VT)
• High school diploma or GED preferred
• Must have own transportation and reliable commute to the facility
• Ability to work 3:00 PM - 11:00 PM shifts with every-other-weekend commitment
• Background check, immunizations, and TB testing as required by facility policy
• Strong communication skills and a team-oriented attitude
• All requested time-off (RTO) must be approved prior to start date

Education

• High school diploma or GED (preferred)
• Completion of an approved LNA program and current LNA certification

Knowledge & Skills

• Skilled in assisting with ADLs and basic patient care
• Comfortable taking and documenting vital signs accurately
• Knowledge of infection control best practices
• Basic charting and documentation skills
• Ability to follow care plans and escalate changes appropriately
• Good time management, observation, and interpersonal skills
