Roles & Responsibilities

• Provide compassionate, hands-on nursing care to senior residents with dignity and respect
• Perform routine assessments and monitor changes in condition, escalating concerns when needed
• Administer medications and treatments accurately while following care plans
• Document care thoroughly and communicate clearly with the interdisciplinary team
• Support activities of daily living and encourage resident independence whenever possible
• Build trusting relationships with residents and families to promote a calm, healing environment

Qualifications

• Active LPN licensure (New York state licensure preferred or required based on facility)
• Current BLS/CPR certification or willingness to obtain promptly
• Clinical experience in nursing, ideally with senior care or long-term care populations
• Comfortable with medication administration, documentation, and basic assessments
• Strong communication skills and a team-first attitude
• Up-to-date immunizations and ability to meet facility onboarding requirements

Education

• LPN diploma or equivalent practical nursing program completion

Knowledge & Skills

• Skilled in senior care principles and resident-centered approaches
• Proficient in medication administration and wound care basics
• Accurate clinical documentation and charting skills
• Ability to recognize and report changes in resident condition
• Strong interpersonal skills for working with residents, families, and care teams
• Time management and prioritization in a fast-paced care setting
