Roles & Responsibilities

• Implement occupational therapy treatment plans that help residents regain independence and confidence
• Collaborate closely with licensed Occupational Therapists and the interdisciplinary care team to ensure holistic, patient-centered care
• Maintain compliance with facility policies, Medicare guidelines, and regulatory standards, ensuring the highest quality of care
• Document progress accurately in Net Health, contributing to seamless continuity of care

Qualifications

• Active OTA License required
• BLS/BCLS certification required
• SNF experience preferred, but mentorship is available for those with 0-1 year experience
• Strong documentation and time-management skills
• A flexible, collaborative mindset ready to adapt to a dynamic environment

Knowledge & Skills

• Occupational Therapy treatment planning and implementation
• Exceptional communication and teamwork abilities
• Proficiency in documentation, including Net Health
• Compliance with healthcare regulations and facility policies
• Time management in a fast-paced skilled nursing setting
• A passion for rehabilitation and patient advocacy
