Roles & Responsibilities

• Provide compassionate daily care to residents in a transitional rehab center, including assistance with bathing, dressing, grooming, and toileting
• Take and record vital signs, and report any changes in condition to the nursing team
• Help residents with mobility, transfers, and positioning to maintain comfort and safety
• Support dining and nutrition by assisting with meals and documenting intake and output
• Maintain a clean, safe, and welcoming environment for residents and their families
• Collaborate with nurses and therapists to ensure each resident's care plan is followed with dignity and respect

Qualifications

• Active Certified Nursing Assistant (CNA) certification in New Mexico
• Must have completed the original or updated COVID-19 vaccine or present a documented religious or medical exemption
• Ability to pass a background check and provide state or federal issued identification
• Previous experience in rehab, long-term care, or skilled nursing preferred but not required
• Strong communication and teamwork skills to thrive in a fast-paced rehab center

Knowledge & Skills

• Knowledge of resident rights and HIPAA compliance
• Skills in taking vital signs and observing changes in patient condition
• Understanding of infection control practices
• Ability to lift, transfer, and position patients safely
• Compassionate, patient, and dependable nature
