Roles & Responsibilities

• Deliver compassionate, coordinated care through the nursing process while maintaining the highest professional nursing standards
• Supervise CNAs and collaborate with physicians, therapists, and other health care team members to keep every patient on track
• Provide comprehensive initial assessments and ongoing evaluations of physical, psychosocial, environmental, and community resource needs across the lifespan
• Initiate and update the Plan of Care, continually reassessing needs and modifying care as ordered
• Build thoughtful discharge plans alongside patients, physicians, caregivers, and the full care team
• Complete accurate OASIS data and documentation that meets all regulatory guidelines
• Assess the educational needs of patients, families, and caregivers and deliver teaching strategies that truly stick
• Promote a clean, safe environment of care by putting SAFE error prevention habits into practice every day
• Protect patient privacy and confidentiality at all times by upholding the Standards of Conduct
• Apply current Federal, State, and Agency regulations to every patient interaction

Qualifications

• Active Illinois RN license, active at the time of submission, with a copy of your license included
• Home Health experience is required, and strong Med Surg backgrounds with some home health exposure are welcome too
• Active BLS certification, or the willingness to obtain one
• Valid driver's license and current car insurance for those home visits
• OASIS and start of care experience is strongly preferred, so definitely highlight it
• COVID and FLU vaccination required, as declinations cannot be approved, so include your vaccination status
• Two supervisory references ready to go
• Up-to-date skills checklist within the last year
• All certifications current and active at submission
