Roles & Responsibilities

• Provide compassionate home health care to patients with CHF, COPD, wounds, and IV-PICC lines
• Educate patients and families on disease management and self-care techniques
• Coordinate with interdisciplinary teams to ensure seamless transitions from hospital to home
• Document care plans and progress notes using Cerner charting system
• Manage RT equipment such as vents, CPAP, BiPAP, and wound vacs
• Respond to patient needs and emergencies during scheduled shifts
• Assist with medication administration and wound care
• Maintain accurate patient records and billing points

Qualifications

• BLS certification
• Valid driver's license and insurance
• Minimum 2 years of home health nursing experience
• Strong patient education skills
• Ability to work independently and as part of a team
• Excellent communication and documentation skills
