Roles & Responsibilities

• Provide high-quality physical therapy treatments to residents in a skilled nursing setting
• Collaborate with physical therapists to develop and implement individualized treatment plans
• Guide patients through therapeutic exercises to improve mobility, strength, and balance
• Monitor and document patient progress, adjusting treatments as needed
• Educate patients and families on safe movement and home exercise programs
• Work with the interdisciplinary care team to ensure the best outcomes for every resident

Qualifications

• Active Physical Therapist Assistant (PTA) license in New Hampshire
• Graduate of an accredited PTA program
• Current BLS/CPR certification
• Experience in skilled nursing, rehabilitation, or long-term care is a plus
• Excellent communication and teamwork skills
• Must pass a background check and meet facility badging requirements

Knowledge & Skills

• Knowledge of therapeutic exercise, gait training, and modalities
• Familiarity with electronic medical records and documentation
• Strong patient assessment and problem-solving skills
• Ability to adapt to diverse patient needs and conditions
• Compassionate, patient-centered approach to care
• Reliable and organized with attention to detail
