Roles & Responsibilities
- Evaluate and treat SNF patients
- Collaborate with the interdisciplinary team to develop and implement patient care plans
- Complete accurate and timely documentation in Net Health per facility standards
Qualifications
- Active CA OT License (or in process under 60-day rule)
- SNF experience preferred, but new graduates welcome with mentorship available
- Strong documentation and time-management skills
- Flexible and collaborative mindset
