Roles & Responsibilities

• Provide compassionate, skilled nursing care to home health patients, ensuring their safety and comfort
• Administer medications and treatments per physician orders and clinic protocols
• Document patient assessments and care using Cerner charting to maintain accurate records
• Coordinate care with the interdisciplinary team to support patient goals and transitions
• Educate patients and families on care plans, medications, and wound care to promote independence
• Monitor patient progress and promptly report changes to the care team to ensure timely interventions
• Follow infection control procedures and best nursing practices to protect patients and staff

Qualifications

• Valid LPN/LVN license (eligible to submit license) — required
• BLS (Basic Life Support) certification — required
• Minimum 2 years of nursing experience (as listed in the job details)
• Experience in Home Health and familiarity with post-acute care preferred
• Charting experience with Cerner preferred
• Ability to complete 12 hours of orientation as required

Education

• Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN) — state licensure required
• BSN not required (per job details)

Knowledge & Skills

• Strong clinical assessment and patient monitoring skills in a home health setting
• Medication administration and documentation accuracy
• Proficiency with Cerner or electronic health record charting
• Excellent communication and patient/family education abilities
• Time management and ability to coordinate care across providers
• Basic wound care and post-acute nursing knowledge
• Professionalism, empathy, and a patient-first approach
