Roles & Responsibilities

• Provide direct patient care in the home setting, administering wound care and managing ostomy supplies
• Perform PICC line care, oxygen therapy, and nebulizer treatments
• Document care accurately using HomeCare HomeBase on Samsung tablets
• Collaborate with interdisciplinary team members to optimize patient outcomes
• Maintain up-to-date charting and follow care plans
• Support patients with cardiac, CHF, and pneumonia conditions

Qualifications

• BLS certification and valid driver’s license
• Insurance coverage required
• 2+ years experience in home health nursing
• Proficiency with HomeCare HomeBase EHR
• Experience with wound care, wound vacs, ostomy care, PICC line care, oxygen and nebulizer therapy
• Ability to manage up to 7 routine visits per day
