Roles & Responsibilities

Provide compassionate, high-quality patient care within the LPN scope
Administer medications and treatments accurately and safely
Monitor patient status and promptly report changes to the RN or provider
Assist patients with activities of daily living while preserving dignity
Document assessments, interventions and patient responses clearly and timely
Collaborate with the interdisciplinary team to support individualized care plans

Qualifications

Current, active LPN license in Pennsylvania or eligibility to obtain one
Current BLS/CPR certification (or willingness to obtain upon hire)
Comfortable working in long-term care, skilled nursing, or similar settings (preferred)
Strong communication skills and the ability to work well on a team
Reliable, punctual, and committed to patient-centered care

Education

LPN diploma or vocational certificate
High school diploma or GED

Knowledge & Skills

Medication administration and accurate documentation
Fundamental patient assessment and vital sign monitoring
Basic wound care and infection control practices
Familiarity with electronic medical records (EMR) and charting
Time management, prioritization, and clinical problem-solving
