Roles & Responsibilities

Provide compassionate, patient-centered care with attention to detail
Administer medications and treatments accurately and safely
Perform thorough assessments and report changes in condition
Document care and maintain accurate patient records in the charting system
Collaborate with nurses and interdisciplinary team members to deliver coordinated care
Assist with activities of daily living and promote patient comfort and dignity
Adhere to infection control and safety protocols to protect patients and staff

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain upon hire)
Completion of an accredited Practical Nursing program
Strong clinical judgment, good communication skills, and a team-first attitude
Previous experience in long-term care, rehab, or acute care preferred but not required

Education

Diploma or certificate from an accredited Practical Nursing program (LPN)

Knowledge & Skills

Medication administration and documentation best practices
Basic patient assessment and vital signs monitoring
Familiarity with electronic health records and charting
Infection prevention and control measures
Effective patient and family communication
Time management and prioritization in a fast-paced setting
