Roles & Responsibilities

• Provide direct, compassionate patient care including assistance with activities of daily living
• Administer medications and treatments per physician/RN orders and facility policies
• Monitor and record vital signs and patient status, escalating concerns promptly
• Maintain accurate, timely documentation in the patient chart and electronic records
• Collaborate with nurses, providers, and interdisciplinary team members to support care plans
• Support a safe, clean environment and follow infection control procedures
• Build rapport with patients and families to promote comfort and understanding

Qualifications

• Licensed Practical Nurse (LPN) credential — active or eligible in the state of practice
• Current Basic Life Support (BLS) or CPR certification (as required by facility)
• Ability to pass employer background checks and required health screenings
• Strong communication skills and a team-oriented approach to patient care
• Dependable, organized, and able to follow established policies and procedures

Education

• Completion of an accredited Practical Nursing program or equivalent diploma

Knowledge & Skills

• Medication administration and basic clinical assessment
• Comfortable with charting and documentation in electronic health records
• Skilled in patient hygiene, mobility assistance, and ADL support
• Knowledge of infection control standards and safe patient handling
• Ability to prioritize tasks and respond calmly in fast-paced situations
