Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope of practice
Administer medications and treatments safely and accurately
Monitor and record vital signs and patient status, reporting changes promptly
Document patient care in the electronic health record with attention to detail
Collaborate closely with RNs, CNAs and interdisciplinary staff to deliver coordinated care
Assist with admissions, transfers and discharges to ensure smooth transitions
Support infection control practices and maintain a safe care environment
Contribute to quality improvement initiatives and resident-centered care

Qualifications

Valid LPN license in Pennsylvania required
Current BLS/CPR certification preferred or obtained upon hire
Previous experience in long term care, skilled nursing or similar setting preferred
Strong communication and teamwork skills
Ability to perform duties such as medication administration, patient assessments and documentation

Education

Graduate of an accredited Practical Nursing program or equivalent vocational training

Knowledge & Skills

Medication administration and safe handling
Basic patient assessment and nursing observation
Electronic health record (EHR) documentation
Infection prevention and control practices
Time management and prioritization in a fast-paced care setting
Effective interpersonal communication with residents and team members
