Roles & Responsibilities

• Provide direct patient care and monitor vital signs during night hours
• Administer medications and document accurately in the electronic health record
• Assist with activities of daily living and ensure patient comfort
• Communicate changes in patient condition to RN and healthcare team promptly
• Maintain a safe, clean, and compliant clinical environment

Qualifications

• Current Pennsylvania LPN license in good standing
• Basic Life Support (BLS) certification
• Minimum 6 months of acute care or long‑term care experience preferred
• Reliable transportation to CVN Crestview Center
• Ability to work overnight shifts and adapt to a fast‑paced environment

Knowledge & Skills

• Patient assessment and vital sign monitoring
• Medication administration and documentation
• Infection control and safety protocols
• Effective communication with patients, families, and interdisciplinary team
• Electronic health record (EHR) proficiency
