Roles & Responsibilities

• Provide direct patient care, including assessments, medication administration, and treatments
• Monitor and document patient vital signs and condition changes
• Collaborate with physicians and healthcare team members to develop and implement care plans
• Educate patients and families on health management and post-discharge care
• Maintain accurate and detailed patient records in compliance with facility policies
• Supervise and support certified nursing assistants to ensure quality care

Qualifications

• Current, valid LPN or RN license in Pennsylvania
• Current BLS/CPR certification
• Previous experience in long-term care, skilled nursing, or rehabilitation settings preferred
• Strong clinical judgment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work varied shifts as needed
