Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and wound care
• Monitor and document patient vital signs and health status changes
• Collaborate with registered nurses and physicians to implement care plans
• Educate patients and families on health management and treatment plans
• Ensure a safe and clean environment by adhering to infection control protocols

Qualifications

• Current and valid Licensed Practical Nurse (LPN) license in New York State
• BLS certification from the American Heart Association
• At least 1 year of recent LPN experience in a long-term care or similar setting preferred

Education

• Graduate of an accredited practical nursing program

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Proficient in electronic medical records (EMR) systems
• Ability to work independently and as part of a multidisciplinary team
