Roles & Responsibilities

• Evaluate patients in their home environment to develop personalized treatment plans
• Provide skilled physical therapy interventions to help patients regain mobility and independence
• Collaborate with physicians, caregivers, and the care team to ensure continuity of care
• Document patient progress accurately in Epic (preferred)
• Educate patients and families on therapeutic exercises and fall prevention
• Adapt treatment approaches to each patient's home setting and lifestyle

Qualifications

• Active Physical Therapist license in Iowa (or willingness to obtain)
• Current BLS certification (AHA)
• Home health or outpatient experience preferred, but new grads with strong clinical rotations are welcomed
• Ability to work 40 hours per week with weekend availability as needed
• Epic experience preferred but not required

Knowledge & Skills

• Strong clinical reasoning in physical therapy assessment and treatment
• Comfort with delivering care in home-based settings
• Excellent documentation skills and experience with EHR systems like Epic
• Ability to work independently and manage a caseload
• Compassionate communication with patients, families, and team members
