Roles & Responsibilities

• Provide compassionate daily care to patients in an acute rehabilitation setting
• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
• Monitor and report changes in patient condition to nursing staff
• Help with mobility, transfers, and positioning to keep patients safe
• Document care provided and maintain accurate patient records
• Support patients and families with empathy and clear communication
• Collaborate with the care team to create a positive healing environment

Qualifications

• Active Certified Nursing Assistant (CNA) certification in Indiana
• Current BLS CPR certification (American Heart Association)
• Previous experience in acute care, rehab, or hospital setting preferred
• Ability to work flexible per diem shifts, including some weekends and holidays
• Strong communication and teamwork skills

Knowledge & Skills

• Knowledge of patient care techniques and infection control practices
• Skill in taking vital signs and recording patient information
• Ability to lift, transfer, and support patients safely
• Strong observation and reporting skills
• Compassionate, patient-centered approach
