Roles & Responsibilities

• Coordinate comprehensive case management activities for patients
• Collaborate with interdisciplinary teams to develop individualized care plans
• Assess patient needs and monitor progress
• Ensure compliance with HCA policies and procedures
• Document patient interactions and outcomes accurately
• Provide compassionate support to patients and families

Qualifications

• 2+ years of case management experience in acute care settings
• Proficiency with Meditech and Midas EMR systems
• HCA experience preferred
• Strong communication and organizational skills
• Ability to work flexible shifts, including alternating weekends
