Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Perform comprehensive assessments and develop individualized care plans
• Administer medications and treatments safely and according to policy
• Monitor resident condition, document changes, and escalate concerns promptly
• Maintain infection prevention practices and ensure immunization compliance
• Complete required Trinity competency testing and participate in ongoing skills validation
• Collaborate with interdisciplinary teams to support resident quality of life
• Support accurate, timely documentation and license/certification verification

Qualifications

• Current Registered Nurse (RN) license (license/certificate verification required)
• Basic Life Support (BLS) certification required
• Experience or comfort working in long-term care or skilled nursing environments
• Completion of Trinity Specialty Specific Competency Testing as required
• Ability to complete required health screenings and immunizations (Fit Test, PPD/IGRA, MMR, Hepatitis B, Varicella, Tdap, Influenza, COVID-19, Trinity 10 panel drug screen, physical/health statement)
• Strong communication, assessment, and documentation skills

Knowledge & Skills

• Knowledge of long-term care clinical practices and resident-centered care
• Skilled in medication administration, wound care basics, and routine assessments
• Familiarity with infection control, immunization requirements, and health screening processes
• Comfort with competency testing protocols and respirator/fit testing procedures
• Able to work collaboratively with nursing assistants, therapy staff, and leadership
