Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Perform thorough assessments and develop individualized care plans
• Administer medications and treatments safely and accurately
• Document clinical findings and charting clearly and timely in the resident record
• Collaborate with interdisciplinary team members to coordinate care and transitions
• Monitor resident condition and escalate changes to the care team as needed
• Support infection prevention practices and maintain required health screenings
• Participate in competency testing and ongoing education to maintain high standards of care

Qualifications

• Active Registered Nurse (RN) licensure with ability to verify license and certifications
• Current BLS certification (required)
• Experience or comfort working in long-term care settings preferred
• Completion of required Trinity specialty competency testing and drug screening as part of onboarding
• Up-to-date occupational health requirements: fit test, 2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19 and Influenza vaccine or appropriate exemptions
• Ability to pass respirator evaluation and color vision testing as required by the facility

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Knowledge of long-term care resident needs and chronic condition management
• Infection control practices and ability to follow facility protocols
• Clear, compassionate communication with residents, families, and team members
• Comfortable with electronic documentation and basic computer skills
• Ability to prioritize, multitask, and adapt in a fast-paced care environment
