Roles & Responsibilities
- Provide compassionate, hands-on physical therapy assistance to residents in a skilled nursing facility
- Assist the physical therapist with implementing and progressing individualized treatment plans
- Document patient progress accurately and timely in the medical record
- Perform safe patient transfers, mobility training, and therapeutic exercises
- Collaborate with nurses, occupational therapists, speech therapists, and support staff to deliver coordinated care
- Educate residents and families on mobility goals, safety strategies, and home exercise programs
- Maintain therapy equipment, ensure infection control, and support a safe treatment environment
Qualifications
- Licensed or license-eligible Physical Therapist Assistant (PTA) — Indiana licensure or eligibility preferred
- Graduate of an accredited PTA program or equivalent training
- Any level of experience considered, including new graduates (job indicates new grads welcome)
- Current CPR/BLS certification or willingness to obtain
- Strong communication skills and ability to work well in an interdisciplinary team
- Comfortable working in a skilled nursing facility (SNF) environment and with older adult populations
Education
- Graduation from an accredited PTA program preferred; open to new graduates
Knowledge & Skills
- Familiarity with physical therapy interventions for mobility, balance, and strengthening
- Knowledge of safe patient handling and transfer techniques
- Skilled documentation and charting of patient progress
- Ability to follow plan of care and modify treatments under PT supervision
- Good clinical judgment, time management, and organizational skills
- Experience or comfort working in a SNF setting and collaborating with nursing staff
