Roles & Responsibilities

• Provide compassionate care to residents in a long-term care setting
• Administer medications accurately and on schedule
• Monitor vital signs and report changes to the healthcare team
• Collaborate with RNs and other staff to ensure optimal patient outcomes
• Assist with daily living activities and promote independence
• Document patient care thoroughly in electronic health records

Qualifications

• Active Vermont Licensed Practical Nurse (LPN) license
• Current BLS certification
• Minimum of 1 year of LPN experience, preferably in long-term care
• Comfortable working evening shifts and adaptable to a new environment

Knowledge & Skills

• Strong medication administration and patient assessment skills
• Proficiency in electronic health records
• Knowledge of infection control practices
• Excellent communication and teamwork abilities
• Compassionate patient-centered approach
