Roles & Responsibilities

• Assess and evaluate patients' speech, language, cognitive-communication and swallowing needs using evidence-based practices
• Develop individualized treatment plans and measurable goals that drive functional progress
• Deliver skilled therapy sessions and adapt interventions to each patient's pace and needs
• Document clinical findings, progress notes, and discharge summaries accurately and timely
• Collaborate with interdisciplinary team members (PT, OT, nursing, case managers) to coordinate patient-centered care
• Educate patients and families on strategies to support carryover and maximize outcomes

Qualifications

• Active state SLP license (Pennsylvania) or eligibility to obtain licensure
• ASHA CCC-SLP preferred or working toward certification
• Experience or strong interest in inpatient/rehabilitation or skilled nursing populations
• Comfort working a full-day schedule across all days of the week as required by the contract
• Current BLS/CPR certification preferred or willing to obtain
• Strong clinical documentation skills and familiar with common therapy software

Education

• Master's degree in Speech-Language Pathology (required)

Knowledge & Skills

• Skilled in assessment and treatment of adult speech, language, cognitive-communication and dysphagia
• Knowledge of evidence-based therapy approaches and functional goal writing
• Excellent communication and patient/family education skills
• Interdisciplinary teamwork and care coordination experience
• Ability to prioritize caseload and manage time effectively
• Familiarity with clinical documentation standards and regulatory requirements
