Roles & Responsibilities

• Perform thorough evaluations to assess patients’ speech, language, and swallowing needs
• Deliver daily treatments tailored to each patient’s goals and progress
• Develop and manage discharge plans to ensure safe transitions and continuity of care
• Document clinical encounters and progress notes accurately and timely
• Collaborate with the interdisciplinary team to optimize patient outcomes
• Meet and maintain the therapy productivity requirement and follow CRG policies and procedures

Qualifications

• Active SLP credential/licensure as required by state regulations (ensure New Mexico eligibility)
• 1–3 years of clinical experience (prior SNF experience strongly preferred)
• Comfortable with evaluations, daily treatment plans, and discharge planning
• Ability to meet a productivity requirement of 90% and follow facility policies
• Professional presentation and adherence to dress code (black scrubs)
• Clear communication skills and the ability to work well within a multidisciplinary team

Knowledge & Skills

• Skilled in adult dysphagia and language/cognitive therapy techniques
• Experience with SNF workflows, documentation, and discharge processes
• Strong clinical reasoning and progress-driven treatment planning
• Time management to balance evaluations, treatments, and documentation
• Collaborative approach to working with nursing and rehab staff
• Familiarity with productivity tracking and meeting performance targets
• Patient-centered communication and family education
