Roles & Responsibilities

• Coordinate comprehensive care plans for patients in the ED and NICU
• Collaborate with interdisciplinary teams to ensure seamless transitions
• Advise patients and families on discharge planning and community resources
• Document interventions and outcomes in Epic
• Maintain up‑to‑date knowledge of best practices in social work

Qualifications

• Bachelor of Social Work (BSW) or Master of Social Work (MSW) degree
• Minimum 2 years of clinical social work experience
• BLS certification required
• Proficiency with Epic EMR
• Strong communication and organizational skills
