Contract Type: Contract

Shift Type: 8-hour shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, rotating weekends

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop daily patient lists and review patient charts
Assess patient needs and initiate goals of care discussions
Educate patients and families on disease process, advance directives, and POLSTs
Hold family meetings and provide end-of-life support and discussions
Attend daily rounds and collaborate with the interdisciplinary team
Carry a caseload of 5-6 new patients and 3-4 follow-ups per day

Qualifications

2-5 years of recent experience in palliative and/or hospice setting
Strong acute care clinical thinking and communication skills
Flexibility and strong organizational skills a must
Desire to serve patients, families, and team members
BLS certification required
