Shift Type: Flexible 8-hour shifts, Monday-Friday or Tuesday-Saturday

Hours per Week: 40

Candidate Pay Rate: $77 per hour

Roles & Responsibilities

Provide comprehensive home health nursing care including patient assessment, wound care, and IV therapy
Manage patients with CHF, COPD, and complex wound care needs, including wound vacs and PICC lines
Coordinate care with physicians and interdisciplinary team to prevent rehospitalization
Complete start of care, recertification, and routine visits with a point-based productivity model
Educate patients and families on disease management and equipment use

Qualifications

Active RN license in California
BLS certification required
Valid driver's license and auto insurance
Experience with HomeCare HomeBase (HCHB) preferred
Strong clinical skills in home health setting

Knowledge & Skills

Patient assessment and care planning
Wound care and wound vac management
IV therapy and PICC line maintenance
CHF and COPD management
Excellent communication and documentation skills
