Roles & Responsibilities

• Perform comprehensive patient assessments to develop individualized care plans
• Coordinate and implement care across disciplines to keep participants safe at home
• Lead discharge planning and intensive medical management for recently discharged patients
• Conduct home visit assessments as needed and document mileage via Concur
• Apply InterQual and Milliman criteria to ensure appropriate utilization and approvals
• Collaborate closely with social workers and physicians to support cohesive decision making
• Monitor and evaluate care plans continuously for clinical and financial appropriateness
• Maintain accurate documentation in EPIC and participate in utilization review activities

Qualifications

• Active Registered Nurse (RN) — WA license pending accepted
• Registered Nurse Case Manager (RNCM) experience or equivalent case management background
• Minimum 2 years of nursing experience (per job requirement)
• BLS certification preferred
• Experience with geriatric population highly preferred
• Familiarity with EPIC, InterQual, Milliman guidelines, and Concur is a plus
• Comfort performing community/home visit assessments and driving to participant homes
• Team player who enjoys interdisciplinary collaboration and care coordination

Knowledge & Skills

• Care coordination and discharge planning best practices
• Utilization review and application of InterQual and Milliman criteria
• Patient assessment and care planning for older adult populations
• Proficiency with EPIC charting and electronic documentation
• Expense and mileage tracking using Concur
• Strong communication and interdisciplinary team collaboration
• Understanding of CMS regulatory expectations relevant to PACE programs
• Ability to manage case loads and prioritize follow-up effectively
