Roles & Responsibilities
- Conduct physical therapy assessments and interventions
- Develop and implement patient treatment plans
- Collaborate with nursing staff and other healthcare professionals
- Provide high-quality patient care in a home health setting
Qualifications
- Physical Therapy degree and licensure
- BLS certification
- Home Health experience, including SOC visits
- Ability to work independently with minimal supervision
Education
- Physical Therapy degree from an accredited program
Knowledge & Skills
- Physical therapy techniques and best practices
- Home health care principles and guidelines
- EMR systems, specifically HCHB - HomeCare HomeBase
- Strong communication and interpersonal skills
