Roles & Responsibilities

• Deliver comprehensive PT assessments and individualized treatment plans for post‑acute patients
• Facilitate daily therapeutic exercises to promote recovery and independence
• Collaborate closely with interdisciplinary teams to optimize patient outcomes
• Perform transfers and assist with mobility to ensure patient safety
• Document progress and adjust interventions based on patient response

Qualifications

• CA Physical Therapy license
• AHA BLS certification
• Minimum 2 years of post‑acute or skilled nursing experience
• Proficiency with Cerner EMR
• Strong communication and teamwork skills
