Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct physical therapy assessments and treatments in home health settings
Develop and implement patient care plans
Collaborate with healthcare teams to ensure comprehensive care
Perform start of care, recerts, and resumption of care visits
Complete daily visits and achieve productivity goals

Qualifications

Current ND Physical Therapy License
BLS certification
Home Health experience preferred
OASIS training a plus
Valid Driver's License and Auto Insurance

Education

Physical Therapy degree from an accredited program

Knowledge & Skills

Physical therapy techniques and best practices
Home health and hospice care principles
OASIS data collection and reporting
Effective communication and interpersonal skills
